Forms That Require Notarization
The following forms must be downloaded, printed, and signed in the presence of a notary:
- Authority for Release of Information
- Physical Agility and Strength Test Agreement
Applicants must bring a valid photo ID when signing these documents. Signed and notarized forms must be submitted in person to the Bedford County Sheriff's Office (1345 Falling Creek Rd., Bedford).
Notary Options
Bedford County Sheriff’s Office: A notary is available during regular business hours. Applicants may complete notarization and submit all paperwork in one visit.
Other Notary Locations: Any licensed notary public may be used, including those at banks, law offices, shipping centers, or public libraries.
